At the workplace, the employer did not ensure the energy control procedures developed were utilized by employees that were engaged in maintenance and service activities where the unexpected energizing, start up or release of stored energy could occur and cause injury.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a.  At the workplace, employees were not protected from hazards of rotating parts while manually starting and stopping the slide press, thereby exposing employees to a caught-in hazard.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  a.  At the workplace, the slide press machine had a barrier guard that was not secured and easily removed therefore exposing employees to an amputation hazard.  b.  At the workplace, mechanical power press #2 had a barrier guard that was not secured and easily removed therefore exposing employees to an amputation hazard.  c.  At the workplace, mechanical power press #5 had a barrier guard that was not secured and easily removed therefore exposing employees to an amputation hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a.  At the workplace, the continuous straightening machine for press #1 had a chain and sprocket that was not enclosed exposing employees to an caught-in hazard. The unguarded chain and sprocket was approximately 40 inches from the floor level.  b.  At the workplace, the rewind oscillator had a chain and sprocket that was not enclosed exposing employees to an caught-in hazard.  The unguarded chain and sprocket was approximately 24 inches from the floor level.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a.  At the workplace, the employer did not guard the point of operation of the continuous straightening machine for press #1 exposing employees to an amputation hazard.  b.  At the workplace, the employer did not guard the point of operation of the continuous straightening machine for press #2 exposing employees to an amputation hazard.  c.  At the workplace, the employer did not completely guard the point of operation of mechanical power press #2 exposing employees to an amputation hazard.  Champion Strapping was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 752441, citation number 1, item number 2 and was affirmed as a final order on February 20, 2013, with respect to a workplace located at 1819 Walcutt Ave, Columbus, Ohio 43228.
