Ladder sections shall be offset from adjacent sections, and landing platforms shall be provided at maximum intervals of 50 feet (15.2 m).    On or about November 11, 2013, employees were utilizing a fixed ladder with a climbing distance greater than twenty-four feet without any ladder safety device, self-retracting lifeline, cage, well, or multiple ladder sections.
