On or about December 2, 2013 at the above addressed site. An employee climbs a fixed ladder on bin number 7 that is approximately 60 feet high with no landing platforms. The employee is exposed to fall hazard(s) with not having adequate rest platforms. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.272(m)(4): The employer did not implement procedures in the grain handling facility for the use of tags and locks which would prevent the inadvertent application of energy or motion to equipment being repaired, serviced, or adjusted, which could cause injury to employees. On or about November 1, 2013 at the above addressed site. The employee greases all components for the grain elevator throughout harvest. The employee turns off the circuit breaker and applies a tag. The company did not implement a procedure in which locks and tags were used. The employee was exposed to a possible startup of equipment causing injuries because locks were not also used. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
