Employees of C and A Roofing were exposed to fall hazards in excess of 11 feet while engaged in roofing activities on the roof of a residential building with no fall protection in place. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a) On or about November 13, 2013, C and A Roofing did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Workers were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
