Location:    552 West 24th Street, New York, NY.  a)    Scrap lumber with protruding nails, materials and other debris and equipment were not removed from the work areas. On or about 11/14/2014.

Location:     552 West 24th Street, New York, NY North West of Site.  a)    Employee worked from a scaffold which did not have guardrails nor was he wearing a fall protection arrest system while exposed to a fall of approximately 12 feet to ground.  On or about 11/14/13.  Location:    552 West 24th Street, New York, NY. South West of Site.   b)     Employees worked from two wood planks which did not have guardrails or a fall protection arrest system while exposed to a fall of approximately 11 feet to ground.  On or about 11/14/13.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   Location:    552 West 24th Street, New York, NY.  a)    Egress ladder was removed and left an opening of 4 feet in width in which an employee fell approximately 14 feet.
