29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) ) On November 15, 2013, at a jobsite located on the corner of Paseo address 5045, Kansas City, MO. An employee employee engaged in roofing work on a steep pitched roof 8-12 pitch, approximately 19 feet above the lower level was exposed to a serious fall hazards without any means of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about November 15, 2013 at 5045 Kansas City Missouri 64128. An employee was exposed to a fall hazard as he worked off of a ladder that was less than 3 feet below the landing surface at the edge of a roof surface approximately 19 feet from the ground.
