29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about November 15, 2013, along the exterior west side of the construction building, two employees were exposed to the hazards of falls while installing insulation on the exterior building wall from an aerial lift at an approximate height of twelve feet above ground level without wearing a body harness and lanyard anchored to the basket of the lift.
