Employees were exposed to falls while working from a wood pallet on the elevated forks of a powered industrial truck.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       The employer failed to properly guard equipment to prevent injuries such as amputations, fractures, dislocation of bones and lacerations when operating the equipment listed below.       1. Bag Machine. Triangle model L6CF serial number DN110075. The front bagging and sealing section of the machine was not guarded.   2. Day Mixer model 006 serial number 8774 had an interlock that was not functioning. The safety device was broken and missing components.   3. American Process Systems Mixer had an interlock that was not functioning. The safety device was broken and missing components.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     Windy Acquisitions, LLC, did not establish a lockout program and machinery was not locked out when performing servicing and maintenance on machinery, including, but not limited to: packing machine and the canning machine.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    Water is added to the forklift battery, exposing an employee to injurious corrosive chemicals, and an eyewash and shower was not available in the work area. Windy Acquisitions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.151(c), which was contained in OSHA inspection number 785101, citation 1, Item 2 and was affirmed as a final order on April 17, 2013, with respect to a workplace located at 454 Country Club Dr., Bensenville IL 60106.
