29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) On or about October 15, 2013, a maintenance mechanic A did not relieve stored energy that had been trapped between the main steam line valve and the 25 mm line steam valve causing scalding water and steam to be released when the valve seal was broken by the employee.
