29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:      a) Jobsite, Chatham Building # 1 roof, Pittsburgh, PA: On or about October 21, 2013, during maintenance activities on the roof, the employer did not ensure that appropriate fall protection systems/devices were used where fall hazards of up to approximately 100 feet were present. The employer had parapet wall, determined to be approximately 24-32 inches in height, around the perimeter of roof of the building, and harnesses, guardrails, safety nets or other fall protection was not in use while employees performed their work.     b. Jobsite, Chatham Building # 1 roof, Pittsburgh, PA: On or about November 6, 2013, during maintenance activities on the cooling tower, the employer did not ensure that fall protection systems/devices were used where employees were exposed to fall hazards of up to approximately 20 feet. Employees climbed up on top of these units and did not have guardrails, harnesses, safety nets or other fall protection in use while they performed their work.
