29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) At 8671 US Hwy 280 E Salem, Alabama: On or about November 13, 2013 and at times prior: the employer failed to ensure the employee on the ground crew cleaning up removed shingles below the workers was wearing a hard hat to protect him from struck-by hazards.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) At 8671 US Hwy 280 E Salem, Alabama: On or about November 13, 2013 and at times prior; the employer failed to ensure his employees involved in roofing activities on a low slope roof were using fall protection exposing them to fall hazards of 19.9 feet to the ground below.
