29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D).    a) Stamping Area: The employer allowed employees to use each of the 5 presses in the facility without providing specific procedural steps for locking out each machine exposing employees to amputation hazards.    b) Welding Area: The employer allowed employees to operate the 250 robots in the facility without providing specific procedural steps for locking out each cell of robots exposing employees to amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the rotating chuck:     a) Weld machine shop area: The employer failed to ensure that a  Jet Drill Press Model # JDP-17MF used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.    b) Weld machine shop area: The employer failed to ensure that a Wilton drill press used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.

Although the drill was designed to be secured to a surface, it was not anchored, which exposed employees to crushed-by/tip over hazards.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:     a) Child Parts area: The employer did not ensure that the flexible electrical cord that powered a portable fan was not spliced. An employee was observed working in the area next to an electric portable fan with a splice to the outer insulation of the flexible cord exposing the employee to shock hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  a) Die Shop: The employer failed to mount and label a portable fire extinguisher for employees working in the area.  The fire extinguisher was sitting on a table with no identification to notify employees of its location.  b) Welding area: The employer failed to label a mounted, portable fire extinguisher for employees working in the D Zone 3 welding area.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place.  a) Stamping Area: The employer allowed employees to operate the 45 Komatsu forklift #S123 (Serial # E-824289) without a nameplate.  The forklift was missing the nameplate which contains all information pertinent to that specific forklift, exposing employees to tip over/crushed by hazards, improper use of equipment hazards, and contact with flammable liquids.
