Scaffold Frames were used as a means of access. On or about 11/08/13 at Augusta, GA: Employees were exposed to fall hazards while performing masonry work on a 2 buck high framed supported scaffold at a height of 12 feet 8 inches. The employer failed to ensure that a ladder was installed to ascend from the ground level to the working level of the scaffold.
