29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: (a) Company Wide: Records were not made available to certify that periodic inspections were being conducted of the energy control procedures at least annually as determined on or about 11/13/2013. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED J.W. Steel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 315228007, citation number 1, item number 3 and was affirmed as a final order on June 19, 2012, with respect to a workplace located at 300 South Caperton Avenue Princeton, WV 24740.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: (a) Pascor Shop: A welder was exposed to occupational noise at 89.4% of the permissible daily noise exposure (eight hour time weighted average of 85 dBA at the 80 dB criterion level) during the 495 minute sampling period on 12/03/2013. For a full eight hour shift this would be equivalent to an average sound level of 89.1 dBA. The employer did not obtain a new audiogram for each exposed employee at least annually, as determined on 11/13/2013. (b) Pascor Shop: A laborer was exposed to occupational noise at 74.34% of the permissible daily noise exposure (eight hour time weighted average of 85 dBA at the 80 dB criterion level) during the 484 minute sampling period on 12/03/2013. For a full eight hour shift this would be equivalent to an average sound level of 87.8 dBA. The employer did not obtain a new audiogram for each exposed employee at least annually, as determined on 11/13/2013. ABATEMENT CERTIFICATION REQUIRED
