29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about 13 November 2013, the welder(s) were observed using the Delco vertical sander in the weld shop.  The sander wheel was not guarded, exposing employees to the cutting hazard(s) presented by the rotating edge of the disc. The exposed edge was approximately 5.5 inches in length.     A.W.T. World Trade Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA Inspection  # 738983,  Citation Number 1, Item Number 20, issued on 05/10/2013, and was affirmed on 6/04/2013, with respect to the workplace located at 4321 N. Knox Ave., Chicago, IL 60641.

29 CFR 1910.253(b)(2)(ii): 29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, and gangways, or where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.    a) On or about 13 November 2013, the employer did not ensure that welding gas cylinders(s) were secured against accidental knocking over or damage from nearby operations.  An Argon gas cylinder present in the work area of employee(s) welding large frames, was not secured either by placement in the assigned space at the back of the welding equipment, via a floor stand or wall bracket. 29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a) The employer did not ensure that, in the painting department, electrical equipment including but not limited to the Dayton Shop Fan was plugged into fixed wiring.  The Dayton shop Fan was plugged into daisy-chained long flexible cables that dropped from the ceiling.    A.W.T. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) The employer did not ensure that where long flexible cables with a multiple outlet terminals (junction boxes) were used to provide electricity to shop tools and equipment, the junction boxes and cables were fitted with strain relief(s).    (1) In the painting department, long flexible cables were connected in series via two junction boxes, providing electricity to the Dayton Shop Fan and/or DeWalt 9 inch Angle Sander/Grinder(s).  Both the ceiling terminal(s) which were plugged into hard-wired outlets as well as at the workstation terminal(s) (the junction boxes) did not have strain relief.    (2) In the welding area, a flexible cord on a retractable spindle, providing electricity to portable handheld grinder(s) was connected to a junction box, without being equipped with strain relief.  The coupling to the junction box was damaged.  A.W.T. World Trade Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.305(g)(2)(iii) which was contained in OSHA Inspection  # 738983,  Citation Number 1, Item Number 24-b, issued on 05/10/2013, and was affirmed on 6/04/2013, with respect to the workplace located at 4321 N. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):     a) The employer did not inspect the flexible electrical cord(s) used for power tools and shop fans to ensure it was free of external defects such as damage to the outer jacket and insulation.  All flexible electrical cords and equipment were energized.      (1) In the grinding department, welder(s) used portable Angle Sander/Grinder tools with an extension cord that had a damaged outer insulation/jacket.  Electrical tape was used to cover the exposed copper wire.        (2) In the welding department, welder(s) used a flexible extension cord on a retractable spindle, providing electricity to portable handheld sander/grinder(s).  The flexible cord had a damaged outer insulation/jacket that had been taped over.        (3) In the welding department, welder(s) used a flexible extension cord on a retractable spindle, providing electricity to portable handheld sander/grinder(s).  The flexible cord had a damaged outer insulation/jacket at the connector.      A.W.T.
