(a) Fine Grading Area (Above Mixing Table): A 4 X 4 metal receptacle box powering a microwave and mixer was not mounted, on or about 11/1/13.   b) Fine Grading Area (Floor): A 4 X 4 metal receptacle box laying on the floor powering a mixer was not mounted, on or about 11/1/13.
