29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) 37 LEFEVER STREET, GETTYSBURG, PA 17325 - An employee working at a height of approximately thirty (30) feet used a piece of wire rope as fall protection by securing it through the hook of his shock absorbing lanyard while the lanyard was attached to the front of the personal fall arrest harness at hip level, thereby exposing the employee to serious injury, on or about November 15, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) 37 LEFEVER STREET, GETTYSBURG, PA, 17325 - An employee engaged in steel erection activities worked at a height of approximately thirty (30) feet, and did not use an adequate fall protection system, thereby exposing the employee to possible injuries, on or about November 15, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
