29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about October 31, 2013, the employer did not ensure that employees using pneumatic nailers during roofing activities were protected from eye or face injuries. Employees were observed using the pneumatic nail guns at the worksite located at State Route 82 and Commons Blvd., Twinsburg, OH.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about October 31, 2013, employees were performing roofing activities on a roof having a slope of 6/12 without the use of fall protection. The height was measured as approximately 18-20 feet. This was located on site at the corner of State Route 82 and Commons Blvd., Twinsburg, OH.
