29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) On or about 11/18/2013 in the High Bay area; where the insulation at the plug end of an extension cord, used to supply power to a portable grinder, was pulled away from the cord grip exposing the conductors. NO ABATEMENT CERTIFICATION REQUIRED
