29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about November 18, 2013, employer did not ensure that scaffold's structural intergrity was inspected by a competent person. This condition exposed employee to fall and struck by hazards exceeding 10 feet.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  On or about November 18, 2013, employer did not ensure that base plates were installed on the scaffold. This condition exposed employee to falls and struck-by hazards exceeding 10 feet.

On or about November 18, 2013, employer did not ensure that a scaffold was plumb to prevent displacement. This condition exposed employee to potential struck-by hazards greater than 10 feet.

29 CFR 1926.451(g)(1)(vii): Each employee is not protected by a personal fall arrest system or guard rail system, while working from a scaffold.     On or about November 18, 2013, the employer did not ensure that employee working on a scaffold was protected by the use of a guardrail system or other adequate protection when working above 10 feet. This condition exposed employee to falls and struck-by hazards.
