29 CFR 1926.20(b)(2): The employer failed to initiate and maintain programs that provided for frequent and regular inspections of the job sites, materials, and equipment were made by competent persons designated by the employer:   At the project or about November 18, 2013, and at times prior thereto, inspections were not completed to ensure compliance with OSHA regulations or the person that completed the inspections was not competent as there were no fire extinguisher monthly inspection, no toilets, and no first aid kits onsite.

29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that was reasonably accessible in terms of time and distance to the worksite, which was available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, was not be available at the worksite to render first aid:   At the project on or about November 18, 2013, and at times prior thereto, no one had first aid training and the nearest medical facility was more than four minutes away.  Employees used heavy earth moving equipment on slopes at the project.
