29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) Exterior Front Entrance -  A laborer was observed cutting out an existing brick wall using a Bosch portable electric chipping hammer and not wearing eye protection, on or about 11/18/2013.    For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:  a) Roof -  Employees were observed climbing a portable metal ladder, 33 ft above the ground,  where the top of the ladder extended 24 inches above the upper landing, on or about 11/18/2013.    For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.
