Employees were allowed to work on and from tubular welded scaffold doing masonry stone work when the walking/working platform was not fully planked.     Condition occurred on or about 11/15/13.

29 CFR 1926.451(e)(1): Scaffold platform more than 2 feet above or below point of access, was not provided with a ladder or direct access from another structure or other equivalent safe access during use:        a) 1245 Westfield Avenue, Clark, NJ 07006. Employees were allowed to work on and from tubular welded scaffold doing masonry stone work when a ladder or direct access from another structure or other equivalent safe access to the walking/working platforms was not provided.         Incident occurred on or about 11/15/14.   Employee fell approximately 20 feet to the ground below while climbing down the scaffold components.

Employees were working on and from tubular welded scaffold in excess of 20 feet above the ground below doing masonry stone work with no fall protection systems in place, i.e. guardrails along all open sides and ends of walking/working platforms.    Condition occurred on or about 11/18/13.
