29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    On or about November 18, 2013, the employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) - (o) when employee exposures were equal or exceeded an 8-hour time weighted average of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) On January 22, 2014, a tank inspector was exposed to an 8-hour time-weighted average sound level of 85.3 dBA, or an equivalent dose of 52.3%.    b) On January 22, 2014, a welder was exposed to an 8-hour time-weighted average sound level of 91.7 dBA, or an equivalent dose of 126.5%.    c) On January 22, 2014, a welder installing baffles was exposed to an 8-hour time-weighted average sound level of 93.8 dBA, or an equivalent dose of 168.8%.    d) On January 22, 2014, a welder installing baffles was exposed to an 8-hour time-weighted average sound level of 92.7 dBA, or an equivalent dose of 145.3%.    e) On January 22, 2014, a turret press operator was exposed to an 8-hour time-weighted average sound level of 88.2 dBA, or an equivalent dose of 78.0%.    f) On January 22, 2014, a shear operator was exposed to an 8-hour time-weighted average sound level of 91.2 dBA, or an equivalent dose of 118.1%.    g) On January 22, 2014, a punch press operator was exposed to an 8-hour time-weighted average sound level of 89.6 dBA, or an equivalent dose of 94.7%.
