29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is:    a) 4-plex apartments: On November 15, 2013 and at times prior thereto the bottom two steps were missing from the stairway that employees used for access to the second floor level.
