29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects: a) GKN Driveline Bowling Green, Inc. - Bowling Green, Ohio: On or about November 13, 2013, employees transferring and handling parts at the P415 Line #2 were not adequately protected from hazards associated with falling objects. Employees were exposed to foot injuries from dropped parts when moving assembled parts to the P415 machine or to nearby dunnage. per ISA changed to: On or about November 13, 2013, employees transferring and handling parts at the P415 Line #2 were not adequately protected from upper foot injuries associated with falling objects. Employees were exposed to hazards from dropped parts when moving assembled parts to the P415 machine or to nearby dunnage. Employees wore standard safety steel toe shoes. Engineering practices were in place but allowed for accidental bumping of the safety switch holding the part in place.
