29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) 1st floor stair access - Employees were not protected from falling through holes on the walking working surface while working on the first level.  This condition exposed employees to a fall hazard of approximately ten (10) feet.  Observed on or about November 18, 2013.    b) 2nd floor stair access - Employees were not protected from falling through holes on the walking working surface while working on the second level.  This condition exposed employees to a fall hazard of approximately twenty (20) feet.  Observed on or about November 18, 2013.    c) 3rd floor stair access - Employees were not protected from falling through holes on the walking working surface while working on the first level.  This condition exposed employees to a fall hazard of approximately thirty (30) feet.  Observed on or about November 18, 2013.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.    a) 3rd floor of building - Employees putting up a fire barrier were exposed to a fall hazard of approximately twenty (20) feet without the use of fall protection.  Observed on or about November 18, 2013.
