29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a) 1820 Christian Street, 4 buck scaffold - The working level of a tubular welded frame scaffold was not fully planked or decked between the front and rear uprights thus exposing employees to a fall hazard of approximately thirteen (13) feet to sixteen (16) feet.  Observed on or about November 18, 2013.    b) 1820 Christian Street, 5 buck scaffold - The working level of a tubular welded frame scaffold was not fully planked or decked between the front and rear uprights thus exposing employees to a fall hazard of approximately thirteen (13) feet to sixteen (16) feet. 29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer:    a) 1820 Christian Street, 4 buck scaffold - A tubular welded frame scaffold with a height to base ratio of more than four to one (4:1) was not restrained from tipping by guying, tying or bracing.  Employees were removing stucco from the 2nd level of a 4 buck scaffold, approximately thirteen (13) to sixteen (16) feet above ground level.  Observed on or about November 18, 2013.      b) 1820 Christian Street, 5 buck scaffold - A tubular welded frame scaffold with a height to base ratio of more than four to one (4:1) was not restrained from tipping by guying, tying or bracing.  Employees were removing stucco from the 2nd level of a 5 buck scaffold, approximately thirteen (13) to sixteen (16) feet above ground level.  Observed on or about November 18, 2013.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.      a) 1820 Christian Street, 4 buck scaffold - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, exposing employees to a fall hazard of approximately thirteen (13) to sixteen (16) feet.  Employees were using the side frame of the scaffold to access and/or egress the working levels.  Observed on or about November 18, 2013.       b) 1820 Christian Street, 5 buck scaffold - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, exposing employees to a fall hazard of approximately thirteen (13) to sixteen (16) feet.
