29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person(s): a) On or about November, 14, 2013, the controlling employer, "The Firm", an Insurance Claims Specialist L.L.C., did not initiate and maintain a safety program which requires regular inspections of jobsites and therefore, did not ensure subcontracted employees, who were required to engage in re-roofing operations, were adequately protected from fall hazards of approximately 16 feet.
