29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o): (a) On or about October 31st, 2013, employees working around the KRB Shearline rebar cutter were exposed to possible entanglement with unguarded conveyor belts and pulleys on the operator's side of the machine in proximity to the operator's station. Abatement certification is required for this item. Use the Certification of Corrective Action Worksheet for this purpose.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) On or about October 31st, 2013, employees in the Batch House control room were exposed to live electrical parts inside an energized (480V) electrical cabinet in that the cabinet's door was open and unattended at the time of the inspection. Abatement certification is required for this item. Use the Certification of Corrective Action Worksheet for this purpose.
