29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    The employer failed to protect employees from potential caught between and amputation hazards associated with unguarded belt and pulley assemblies.  This was most recently evident during the inspection of the facility located at 105 E. Highway 20, Belden, NE 68717, where the employer has employees working and cleaning throughout the facility where in more than one location there are belt and pulley assemblies with inadequate or missing guards.  Instances include but are not limited to:  i.    Employees working and cleaning in and around the inadequately guarded belt and pulley assembly for the auger out of the silver bin and the auger into the elevator leg.  ii.   Employees working and cleaning in and around the inadequately guarded belt and pulley assembly for the augers coming out of bins 1, 2 and 3 on the north side of highway 20.   iii.   Employees working cleaning in and around the inadequately guarded belt and pulley assembly for the augers coming out of bins 1, 2, 3, 4, 5, 6 and 7 on the south side of highway 20.

29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o):      The employer failed to protect employees from potential caught between and amputation hazards associated with unguarded belt and pulley assemblies.  This was most recently evident during the inspection of the facility located at 105 E. Highway 20, Belden, NE 68717, where the employer has employees working and cleaning throughout the facility where in more than one location there are belt and pulley assemblies with inadequate or missing guards.  Instances include but are not limited to:  i.    Employees working and cleaning in and around the inadequately guarded belt and pulley assembly for the auger out of the silver bin and the auger into the elevator leg.  ii. 29 CFR 1910.272(j)(1):    The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces in the grain handling facility:                                                                            The employer failed to protect employees from potential combustible dust hazards associated with operating a grain handling facility without having developed and implemented a written housekeeping program as outlined in this subpart.  This was most recently evident during the inspection of the facility located at 105 E. Highway 20, Belden, NE 68717, where the employer has employees working and cleaning throughout the facility without having established which areas are priority housekeeping areas, cleaning schedules and cleaning methods.

29 CFR 1910.272(m)(3): A certification record maintained of each inspection by the employer in the grain facility was not in accordance with 29 CFR 1910.272(1):    The employer failed to protect employees from potential explosion hazards associated with operating a grain handling facility without maintain a certification record of all maintenance performed and a schedule of all maintenance due.  This was most recently evident during the inspection of the facility located at 105 E. Highway 20, Belden, NE 68717, where the employer has employees working and cleaning throughout the facility without maintaining a certification record of which repairs have been made and when the next ones are due creating the potential for a part failing due to over use.
