29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) 1038 East Passyunk Avenue - Employees performing stucco operations from a tubular welded frame scaffold were exposed to fall hazards of approximately thirteen (13) feet to twenty-six (26) feet without the use of personal fall arrest systems or guardrail systems.  Observed on or about November 13, 2013.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) 1038 East Passyunk Avenue - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, exposing employees to a fall hazard of approximately thirteen (13) to twenty-six (26) feet.  Employees were using the side frame of the scaffold to access and/or egress the working levels.  Observed on or about November 13, 2013.

29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer:  a) 1038 East Passyunk Avenue - Tubular welded frame scaffold with a height to base ratio of more than four to one (4:1) was not restrained from tipping by guying, tying or bracing.  Employees were applying stucco screen between the 2nd and 4th working levels, approximately thirteen (13) to twenty-six (26) feet above ground level.  Observed on or about November 13, 2013.

Observed on or about November 13, 2013.
