(a) Colorado Structures, Inc. @ 2100 Delgany St., Denver, CO 80216; On and before November 19th, 2013, the exposing employer did not ensure employees were instructed in the recognition and avoidance of unsafe conditions associated with equipment operation and struck-by hazards. The employer did not effectively instruct employees in the correct operating procedures of the Bobcat T250. The operator did not ensure employees maintained safe distances from the vehicle in operation. The condition resulted in an employee being struck-by the skid steer while attempting to communicate with the operator.
