29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection: (a) On or about November 8th, 2013, at a work site located at 830 N. Boonville Ave. in Springfield, MO, employees were exposed to electrical shock hazards while using electrical tools receiving power through an extension cord run from a permanent building outlet which was not GFCI protected. Abatement certification is required for this item. Use the Certification of Corrective Action Worksheet for this purpose.

Boonville Ave. in Springfield, MO, employees were exposed to electrical shock hazards while using an extension cord that had been run through and pinched between two metal framed doors at the building's east entrance.
