29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or about November 20, 2013, and at times prior thereto, an employee without fall protection on the North side of a residential house was exposed to a fall hazard greater than 6 feet, while placing roof decking on a steep roof with a pitch of 6:12. (b) On or about November 18, 2013, and at times prior thereto, an employee without fall protection on the North side of a residential house was exposed to a fall hazard greater than 6 feet, while placing roof decking on a steep roof with a pitch of 6:12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about November 20, 2013, employees working without fall protection on the North side of a residential roof were exposed to a fall greater than 6 feet in height to the unpaved ground below, while placing roof decking on a steep roof with a pitch of 6:12.
