29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:   On or about November 20, 2013, and at times prior thereto, the employer did not ensure that the tines of the GEHL Model RS8-42 rough terrain forklift used as scaffold platforms were at least 18 inches wide, exposing employees to a falling approximately 5 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 20, 2013, and at times prior thereto, an employee working on the rooftop of the car wash under construction had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazard of approximately 17 feet.
