The slacked rope used as guardrails was inadequate to prevent falls and was completely missing in some areas.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee uses the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and lifesaving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.    (a) Employee(s) were not required to wear personal flotation devices at all times while working over or near water during "boat haul-out activities" and vessel repair work, exposing them to serious injuries, such as drowning, water inhalation, and exposure.
