29 CFR 1910.23(e)(1):   Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  Work site located at 105 Grandville Avenue, Danbury, NE; Middle Elevator:  The floor hole created by the single person elevator/man lift is guarded on the backside by a guardrail system.  The system does not include an intermediate rail which creates an opening between the top rail and the floor of 38 inches with a width of 33.5 inches.    Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

Due to the lack of proper documentation, the employer appears to have allowed some of the operators training to lapse.      Abatement certification is required for this violation.
