29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Jobsite: Near Lot 418, Estates Drive, Gibsonia, PA 15044: On or before November 20, 2013, employees were exposed to excavation collapse hazards while working to install sanitary water drainage pipe. The excavation was approximately 7 feet 6 inches in depth and did not have a protective system in place. The company, Mark Mashuda Excavating, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 891190, citation number 1, item number 2, and was affirmed as a final order on 5/21/13, with respect to a workplace located at 3937 Kirk Avenue, Allison Park, PA 15101.
