29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches. On or about November 20, 2013 employees of Cenex Harvest States Inc. were exposed to explosion hazards due to the failure of the employer to ensure that all receiving grate openings were 2 1/2 inches or less to prevent the introduction of ferrous material into the elevator leg.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employee. Employees were provided filtering face piece (dust mask) respirators to be used on a voluntary basis were not provided information contained in appendix D of the respiratory protection standard. Cenex Harvest States Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 301556932, citation number2, item number 1, and was affirmed as a final order on July 27, 2009, with respect to a workplace located at 6603 South Concord Street, Davenport, Iowa 52802-2214.
