29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.   On or about November 19, 2013, inside the house an employee was exposed to the hazard of being struck in the head by falling nail guns or stacks  of cut lumber in that he was not wearing a hard hat.

29 CFR 1926.102(a)(1):   Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a.   On or about November 19, 2013, on the roof, an employee using a pneumatic nailer was exposed to an eye injury hazard from flying debris in that he was not wearing safety glasses.  b.   On or about November 19, 2013, on the ground, an employee cutting OSB boards with the Dewalt circular saw was exposed to an eye injury hazard from sawdust and flying debris in that he was not wearing safety glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a.   On or about November 19, 2013, on the roof trusses, employees installing framing and roof sheathing were exposed to 11 and 21 feet fall hazards in that they were not protected by a fall protection system.  b.   On or about November 19, 2013, on the second floor employees working near the stair opening were exposed to a 9 feet fall hazard in that they were not protected by a fall protection system.

On or about November 19, 2013, at the front of the garage, the roof access ladder did not extend 3 feet above the roof eave, exposing employees to a 10 feet and 8 inches fall hazard.
