No materials or equipment may be placed, either permanently or temporarily, within the exit route.    a) In warehouse department - The employer did not keep the exit routes free and unbarred for employees working at the area. The employer blocked the exit routes with parts, pallets, rolls of vinyl papers, and an out of order forklift, exposing employees to entrapment during an emergency.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:  a) In shipping department -  The employer failed to ensure employees operating forklift trucks maintain a clear view of the path of travel, exposing employees to the hazard of being struck-by the load and/or forklift trucks. The operator of a 5.0 Nissan (serial # D459714) forklift truck was moving in the forward direction with 11 stacks of rolls for vinyl wall paper approximately 7 feet high obstructing the operator's view.

Employees were permitted to operate Nissan, Clark, Mitsubishi and Toyota forklift when they were not provided with any training and evaluation, exposing employees to struck-by hazard.
