CT A written respiratory program was not established and implement to ensure that the employee wearing half face negative pressure respirator on a voluntarily basis was trained in the safe and proper use of the respirator, the procedures and means to ensure respirator was kept clean at all times and means to ensure the respirator was properly stored and appropriately maintained.
