29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):      (a)  Pipeline Solutions LLC at 230 S. Downing St., Denver, CO 80210:  On November 16, 2013 the exposing employer did not ensure that an employee working on a ladder inside a vertical wall trench was protected from cave-in.  The employee connected equipment to a backhoe bucket that was located inside the trench near the east end.  The trench wall where the connection was made was undercut.  The depth of the trench at the east end was approximately 7 feet.  This condition exposed the employee to a crushing hazard by excavation wall cave-in.
