29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Barclay Glen construction site - Management employees of the company had not been instructed in the recognition and avoidance of fall hazards, such as employees without fall protection exposed to falls from the 2nd and 3rd floor decks, on or about 11/19/14; b) Barclay Glen construction site - Management employees of the company had not been instructed in the recognition and avoidance of head injuries, such as employees without head protection working from ladders while installing HVAC equipment and exposed to both overhead and lateral hazards to the head, on or about 11/19/14; c) Barclay Glen construction site - Management employees of the company had not been instructed in the regulations applicable to his/her environment to control or eliminate hazards to their employees and subcontractors by use of frequent inspections and requiring the use of head protection and fall protection, on or about 11/19/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
