Bonsal American, Inc.- On or about November 21, 2013, a mixer, required to wear a 3M brand half-face air-purifying respirator during mixing operations, had not been provided fit-testing annually.  Fit-testing was last conducted in 2003.

On or about December 5, 2013, the seal between the tight-fitting respirator facepiece and the employee's face was compromised by facial hair.
