29 CFR 1910.134(f)(2): The employer did not ensure that all employees using a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator, whenever a different facepiece (size, style, model or make) is used, and at least annually thereafter: a. Bonsal American, Inc.- On or about November 21, 2013, a mixer, required to wear a 3M brand half-face air-purifying respirator during mixing operations, had not been provided fit-testing annually. Fit-testing was last conducted in 2003.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: a. Bonsal American, Inc.-A mixer was required to wear a 3M brand half-face air-purifying respirator during mixing operations. On or about December 5, 2013, the seal between the tight-fitting respirator facepiece and the employee's face was compromised by facial hair.
