29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) North Side of the Bridge - Employees worked on the deck of a 10 foot high bridge near an 8 foot wide, missing section of guardrail with only a piece of plastic warning ribbon on the deck to protect them from a fall hazard. This condition was observed on or about 11-21-13. The J D Eckman Inc. company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 112984968, citation number 01, item number 001, and was affirmed as a final order on 08-17-2009, with respect to a workplace located at Spring Road Bridge Replacement, Carlisle, PA.

29 CFR 1926.20(b)(2): The employer did not maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. a) North and South sides of the bridge - The employees used Construction Electrical Products # CPE-2 temporary power boxes to provide GFCI protected power for the employee electrical hand tools. On the day of the inspection four of the GFCI outlets in those boxes did not trip when tested and the boxes had not received a monthly inspection, observed and reported on or about 11-21-13. ABATEMENT CERTIFICATION REQUIRED
