29 CFR 1910.23(c)(1)   Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toe board wherever, beneath the open sides, persons can pass, there is moving machinery, or there is equipment with which falling materials could create a hazard:  a) Horizontal department, for employee(s) who walked on the the overhead storage area above the tool grinding room and above CNC Programming to change air conditioner filters approximately monthly, the overhead storage was not equipped with a standard railing.  The fall hazard was approximately 9 feet.    b) Storage area, for employee(s) who walked on the overhead storage area to access boxes approximately once per month, the overhead storage was not equipped with a standard railing.  The fall hazard was approximately 9 feet.  c) Welding shop, for employee(s) who walked on the overhead record storage area to store and retrieve records approximately 3-5 times per year, the overhead storage was not equipped with a standard railing.  The fall hazard was approximately 9 feet.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:   a) Turning department, JET 20 inch floor drill press (SN 10100536) used by the CNC lathe operator daily lacked a chuck guard. b) Horizontal department, Guardian Power 12-speed heavy duty drill press (SN 45922) used daily to monthly lacked a chuck guard. c) Horizontal department, Wilton 15 inch drill press (250-3100 rpm) used daily lacked a chuck guard. d) Horizontal department, ISO drill press (labeling illegible) used daily lacked a chuck guard. e) Turning department, JET 20 inch floor drill press (SN 10100513) lacked a chuck guard. f) Vertical department, ENCO drill press (SN 302197) lacked a chuck guard. g) Vertical  department, Delta drill press (SN illegible, motor SN FR16623) lacked a chuck guard. h) Vertical department, Delta drill press (DP220) used daily lacked a chuck guard. i) Secondary Operations near the Welding Shop, four Powermatic drill presses used for counter sinking lacked chuck guards. j) Tool room, Powermatic drill press (SN 0155532) used approximately once per week lacked a chuck guard. k) Tool room, Clausing drill press (SN 501698) used approximately once per week lacked a chuck guard. l) Tool room, Clausing 1500 lathe (Machine #2514) used approximately once per week lacked a chuck guard.

Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  a) Horizontal department, a knockout receptacle outlet box on a pendant was used to service the JET 20 inch drill press (SN 10100513).  The knockout receptacle outlet box was not used according to the manufacturer's instructions.  b) Vertical department, a knockout receptacle outlet box on a pendant was used to service an ENCO drill press (SN 302197). The knockout receptacle outlet box was not used according to the manufacturer's instructions.  c) Maintenance shop, a knockout receptacle outlet box on a pendant was used to service a radio, phone and Milwaukee 8 inch wheel bench grinder (SN W4-00).  The knockout receptacle outlet box was not used according to the manufacturer's instructions.
