29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about November 21, 2013; at the center of the jobsite, the employer allowed employees to use a hand held Porta-Band saw that was missing a ground pin. The Porta-Band saw was used to cut threaded rod. The missing ground pin prevented the Porta-Band saw from having a continuous path to ground.
