NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1030(d)(3)(x): Masks in combination with eye protection devices, such as goggles or glasses with solid side shields, or chin-length face shields, were not worn whenever splashes, spray, spatter or droplets of blood or other potentially infectious materials may be generated and eye, nose or mouth contamination can be reasonably anticipated:   a) Dental/medical office: The employer did not ensure that eye protection devices, such as goggles, glasses with solid side shields or chin-length face shields, were used by dental assistants when assisting the dentist in activities that resulted in spraying of blood or other potentially infection materials, such as, but not limited to, drilling and extraction, on or about 11/20/13.

29 CFR 1910.1030(d)(4)(ii)(E): Reusable sharps that were contaminated with blood or other potentially infectious material were stored or processed in a manner that required employees to reach by hand into the containers where these sharps had been placed:  a) Medical/dental office: Employees handled contaminated reusable sharps by hand when placing them in the ultrasonic cleaner without the use of puncture resistant gloves, on or about 11/20/13.
